Kindred Transitional Health Care and Rehabilitation Center of Windsor

Kindred Transitional Care & Rehabilitation of Windsor, located at 581 Poquonock Ave in Windsor Connecticut, first opened its doors in March of 1964 under the name of Mountain View Convalescent Home with 64 beds in private and semi-private rooms. At that time, Mountain View provided a rather special service in that it operated a “day patient” section in which elderly people who wished to spend the day under professional care and supervision and return to their own homes at night. An addition to Mountain View was opened in 1966. Today Kindred Healthcare operates the facility with a total of 108 beds of which 12 are private rooms. The mission of Kindred is dedication to hope, healing and recovery. It is the largest diversified provider of care services in the United States.

Kindred Transitional Care and Rehabilitation – Windsor offers short-term rehabilitative as well as long-term care - a full range of medical, nursing, rehabilitation and social services to treat and support each of their patients and residents. Many of their patients go home after a few weeks and, for those who are unable to return home right away, they provide safe, compassionate care in an environment that fosters independence and dignity.

Kindred is well established in the Windsor community and has partnerships and affiliations with their local hospitals, Chamber of Commerce, the L.P. Wilson Senior Center and John Fitch Court. They believe community involvement is important, and enjoy participating in a variety of events around town, including the Windsor Feud , the Chamber’s Chili Challenge, Business After Hours and the Senior Center’s Early Bird Suppers.

They are improving our clinical capabilities and staffing in order to accept patients with greater medical complexity than a traditional nursing home. This is increasingly important as many higher acuity patients require transitional care before they are ready to return home.
